Periodontal add-on: $25/month (or $300/year) — includes 2 additional cleanings for a total of 4 per year. Recommended for patients with periodontal disease.
Additional child (Family Plan): $26.67/month (or $320/year) per child
Not included: Cosmetic services such as Invisalign, Botox, and Veneers (15% member discount still applies)
